Job Summary
Minnesota Home Health Services is dedicated to providing person-centered support to individuals with disabilities. As a case aide, you will collaborate with developmental disability or long-term care case managers, offering crucial administrative assistance without direct client interaction.
Key Duties
- Assist case managers in compiling information for billing reports.
- Conducted data entry tasks and distributed reports efficiently.
- Maintain and distribute division client and client coverage lists.
- Handle filing, faxing, paperwork, file preparation, mail-outs, and other administrative duties.
- Support case managers in obtaining forms and information from service providers.
- Stay informed about industry changes, DHS regulations, and contractual requirements.
- Perform any additional duties as assigned.
Requirements
- High school diploma required; Associate degree preferred.
- Minimum of one year of post-secondary education OR relevant experience in administrative assistance, office/clerical work, case aide, or a related field.
- Preferred experience in the human services sector.
- Ability to lift and carry up to 30 pounds.
- Proficiency in computer skills, with a demonstrated ability to work independently, meet deadlines, and maintain excellent attention to detail.
- Background clearance is required.
- Practical communication skills in English, both written and verbal.
- Valid driver's license, acceptable driving record, and proof of auto insurance.
- Must be at least 18 years of age.

Job Description
Minnesota Home Health Services seeks a compassionate and dedicated individualized home supports caseworker to join our team in the seven metro counties. Formerly known as an independent living skills (ILS) worker, this role involves supporting adults with disabilities to achieve maximum independence within their communities. Certified nursing assistants (CNAs) are encouraged to apply.
Key Responsibilities
- Adhere to organizational policies and procedures, ensuring all documentation is completed accurately and complies with regulations.
- Maintain clear and effective communication with the individual's support team and supervisor.
- Collaborate with individuals to develop goal-oriented outcomes tailored to their specific needs and preferences.
- Implement customized solutions to help individuals achieve a balanced life that aligns with their priorities and promotes good health for independent living.
- Provide individualized home supports services, assisting and training in various community living service categories.
- Deliver support and training either in-person or remotely, as needed, within the individual's home, family home, or community setting.
Qualifications
- Bachelor's degree in social work, psychology, human services, or a related field is preferred.
- Previous experience working with individuals with disabilities or in a similar supportive role is advantageous.
- Certification as a nursing assistant (CNA) is highly desirable.
- Strong communication skills, both verbal and written.
- Ability to work independently and collaboratively within a team environment.
- Demonstrated empathy, patience, and understanding when working with individuals facing diverse challenges.
- Proficiency in documentation and record-keeping.
- Valid driver's license and reliable transportation.

- Support: Our caseworkers provide cues, guidance, and assistance with daily living tasks to help individuals thrive in their community.
- Training: Our staff offers direct instruction to develop essential community living skills, promoting independence and empowerment.
- Streamlined Service Changes: Through Waiver Reimagine, we've combined services for a more comprehensive approach, ensuring tailored support for each individual's needs and goals.
Successful Applicant Requirements
- Proficient management of a caseload for individualized home supports (IHS) services.
- Clean driving history and reliable transportation.
- Ability to pass a comprehensive background study.
- Engage in purposeful activities with clients and teach them basic home management skills.
- Maintain an open-minded and patient approach in all interactions.
- Possess a friendly and outgoing demeanor, willing to take ownership of the position.
- Outline and encourage personal care and provide updates on mental health.
- Exceptional writing skills for clear documentation of encounters and issues.
- Foster and maintain positive relationships with residents.
Acceptable Driving History Criteria
- No more than two minor moving violations in the past three years.
- No significant violations (such as DUI, careless, reckless, serious speed, or driving while license suspended) in the past five years.

In addition to meeting these requirements, successful candidates must demonstrate constructive communication skills with residents and exhibit diligence and persistence in their duties. The ability to positively influence clients who may initially refuse services is crucial to improving their lives.
