Our Mission Statement

  Is to support people with disabilities as they explore options, participate in the community, and strive towards independence and equality.  

Services We Provide

Housing Transitioning and Stabalization

 Housing Stabilization Services (HSS) provides support to individuals who are on MA, and 18 years or older with a disability.  They can be referred through their waiver case manager, and if they do not have a waiver they will need a professional statement of need and a housing-focused person-centered plan.  A targeted case manager or care coordinator can complete the housing-focused person-centered plan. 

24-Hour Emergency assistance


 On-call counseling and problem solving and/or immediate response for assistance at a person's home due to a health or personal emergency. 


 • Crisis prevention skills 

 • Implementation of positive support strategies  

 

Individualized home supports: with training


Services providing support and training in community living service categories for people 18 years or older who live in their own homes.

 Designed to holistically support a person in their own home and within their community while increasing choices and options of how and where to receive services to live,  work and enjoy life in the community. 



In Home Respite Care

 


This short-term, non-medical service provides a break for the family caregiver and positive social interaction for a  adult needing care. We offer flexible scheduling that is tailored to meet the needs of caregivers.

How to Get 245D Waivered Services


Obtain a Home and Community-Based Services (HCBS) Waiver

  • Contact your County Human Services Office
  • Request a MnCHOICES Assessment


Connect with a social worker


  • Discuss and select which HCBS Waiver best meets your needs
  • Discuss and select services from your specific HCBS Waiver service menu.

Begin with the Coordinated Entry process to get on housing waitlists, connect with your specific county’s Coordinated Entry program.
