Our Services
We provide home and community-based services that support children, adults, and older adults with disabilities in building independence, stability, safety, and connection in their everyday lives.

- Individualized Home Supports with Training
Support focused on skill-building, independence, community participation, and daily living needs. - Individualized Home Supports with Family Training
Training and support for the person and family to help maintain stability, independence, and success at home and in the community. - Individualized Home Supports without Training
Support with community living needs when formal skill training is not required. - Basic Homemaking
Household support to help maintain a clean, safe, and stable living environment as authorized in the person’s service plan. - Adult Companion Services
Non-medical support focused on companionship, supervision, community participation, and meaningful social connection as authorized in the person’s service plan. - Individual Community Living Supports (ICLS)
Support for eligible older adults who need reminders, cues, intermittent/moderate supervision, or assistance to remain safely in their own homes. - 24-Hour Emergency Assistance
On-call support for urgent needs as authorized in the person’s service plan.

Service Areas
We provide services throughout the Metro, Southeast and Northeast communities. Our team supports children, adults, and older adults with disabilities who qualify for home and community-based services through Medical Assistance, county or tribal case management, or Minnesota waiver programs.
