Case Aide

  

HOURS AVAILABLE: Monday - Friday 8am - 4:30pm

JOB SUMMARY: Minnesota Home Health Services provides person-centered services to individuals with disabilities. As a Case Aide, you will work with either Developmental Disability or Long-Term Care Case Managers and assist them in an administrative capacity that does not involve contact with individuals served. 


DUTIES INCLUDE:

  • Help case      managers compile information for billing reports
  • Data entry and      report distribution
  • Maintaining and      distributing division client and client coverage lists
  • Filing, faxing,      paperwork and file preparation, mail-outs, and other administrative duties
  • Assist case      managers with obtaining forms and information from providers
  • Remain abreast      of industry changes, DHS changes, and contractual requirements
  • Other duties as      assigned

REQUIREMENTS:

  • A High School      diploma is required; an Associate degree is preferred
  • One year of      post-secondary education OR administrative assistant,      office/clerical work experience, case aide, or closely related experience
  • Experience in      the Human Services field is preferred but not required
  • Required to be      able to carry/lift 30 pounds
  • Proficient      computer skills, ability to work independently, ability to meet deadlines,      and excellent attention to detail
  • Background clearance
  • Ability to      effectively communicate in English, written and verbal
  • Driver's      license, acceptable driving record, and proof of auto insurance required
  • Must be at least      18 years of age

ILS/IHS Case Worker

Minnesota Home Health Services is hiring an Individualized Home Support Case Worker in the seven metro Counties, Previously Independent Living Skills (ILS) Worker to support adults with disabilities to help them live as independently as possible in the community. CNA's encouraged to apply

  • Adhere to policies and procedures and ensure proper documentation is completed within compliance.
  • Clearly communicate with a person-s team and supervisor.
  • The main objective is to develop goal-oriented outcomes with the individual, using customized solutions to help them discover a life of balance between what is important to them and what will help them maintain good health for independent living.
  • Individualized home supports Services that provide support and/or training in community living service categories ( for people who live in their own home or their family's home.
  • Individualized home supports can be provided in the person-s own home, family-s home, or the community, either in person or remotely.
  • There are three types of individualized home supports.
  • Support: A Case Worker provides a person with cueing, maintenance, guidance, instruction, assistance with coordination of community living activities.
  • Training: Instructional services through which a person receives direct training from a staff member on community living skills identified in the person-s assessment.
  • Training includes skill-building to acquire, retain and improve the person-s experience living in the community.
  • Through the Waiver Reimagine streamlined service changes, this option combines the previous services of independent living skills (ILS) training, supported living services (SLS) - own home, and the previous version of individualized home supports.
  • Requirements for a successful applicant: + Be able to manage a caseload of the Client's needs for IHS services.
  • + Acceptable Driving History and dependable transportation.
  • + Ability to pass a background study.
  • + Participate in purposeful activities with clients.
  • + Manage and teach clients basic skills of managing home effectively.
  • + Ability to remain open-minded and patient.
  • + Must be outgoing and friendly, willing to step in and take ownership of the position.
  • + Outline and encourage personal care as well as mental health updates.
  • + Must be able to write clearly and good at documenting encounters and issues.
  • + Maintain and encourage positive relationships with residents.
  • Acceptable Driving history includes: -No more than two minor moving violations in the past three years-No major violations (DUI, Careless, Reckless, Serious Speed, Driving While License Suspended) in the past five years to be successful in this position, the applicant must be able to constructively communicate with residents, as well as be diligent and persistent.
  • Some clients will refuse services, and we are looking for someone who can change that around to help and improve the lives of those we serve.
